Appointment |
 To schedule an appointment, please call us at (847) 492-3040. One of our patient advocates will be glad to assist you. PLEASE READ THROUGH THE FOLLOWING INFORMATION PRIOR TO YOUR INITIAL VISIT The kind of medicine we practice requires extensive background information. To help us understand how to aid you most fully in regaining your health, we ask that you fill out our 10-part patient questionnaire. The information you provide helps us to personally understand your needs and begin designing a specialized treatment program. We can send you a copy or it can be downloaded from our Web site. Whatever the word "scientific" means, it certainly does not apply to a good part of what a doctor does or should do. For example, two patients could come to our center – same age, weight, height, lab reports, complaints and even the same general history. Yet we and most concerned physicians should be able to detect significant difference between them, which would not appear as part of a purely "scientific" examination. Those differences have to do with the individual nature of the patient, which can often be determined only after and extensive relationship. "Non-scientific" components of a patient’s personality, such as background, lifestyle, stresses, worries and past and future concerns are as much a part of what a physician must be aware of as things such as blood pressure and blood cell count. For example, if we sense that a patient is somewhat apprehensive, the blood pressure reading means something quite different than it appears on the surface. Some items, which you might consider to be unimportant, may be very significant to us. Stressful factors in your life and your developmental and family history are significant and will help us understand you fully. Even the most seemingly innocent piece of information may provide us with an insight we might not have otherwise identified to aid direction in providing you the best possible care. This questionnaire should be filled out and returned to our office at least one week prior to your scheduled appointment if possible. We kindly ask that you mail the questionnaire back, rather than faxing it. If you find a question difficult, please feel free to contact us, and please keep in mind that absolute confidentiality is assured in this matter. We also need certain medical records and laboratory reports. Since our clinical staff reviews this information prior to your appointment, it is critical that we receive it at least a week before your scheduled appointment if possible. The following checklist is meant to provide you with a basic guide to the materials we will need from your medical records. Please contact a Patient Advocate in our office if you have any questions or need our assistance to obtain records.
